A nonpartisan state agency found state health officials launched 90 investigations at the Veterans Home in King over a five-year period.
The Legislative Audit Bureau asked the federal Centers for Medicare and Medicaid Services for more information on about 185 violations.
After investigations, 40 cases were substantiated. Eighteen of them were accusations of abuse between two people living there.
