APPLETON, Wis–Fox Valley healthcare providers are looking for input on the needs of the community. ThedaCare, Advocate Aurora Health, Ascension Health, and Children’s Wisconsin have launched an on-line survey for residents of Outagamie, Winnebago, Calumet, Green Lake, Marquette, Menominee, Shawano, Waupaca, and Waushara counties.
Area health departments are also helping with the survey. Here is a link to the survey.
The survey will be open until January 17th.
